NOTE: Employer plan sponsors are required to file an annual Form 5500 with the DOL if any of the organization’s health and welfare plans had 100 or more employee participants enrolled on the first day of an ERISA plan year. When a Form 5500 is required, the filing is due within 7 months of the last day of the plan year. Employers have the option to file Form 5500s individually – one form per benefit – or as a single, consolidated 5500 including all ERISA benefits if the employer has an ERISA Wrap SPD in place.
